§ 110.02 APPLICATIONS TO BE FILED.
   (A)   Each person subject to the payment of a license fee under this chapter shall file an application with the City Clerk/Treasurer on or before March 31, and on or before May 31, of each year thereafter that such person shall renew or obtain any license subject to the provisions of this chapter. Such applications shall be filed on forms to be furnished by the city, which shall be obtained at the office of the City Clerk/Treasurer.
   (B)   Each application shall show each kind of business, profession, occupation, trade, vocation, calling, or activity engaged in or services rendered; the address of each place of business licensed; the exact name shown or to be shown on the taxpayer's income tax return to the state, the exact number of employees, and such other information as may be considered necessary by the city.
