§ 155.011 FORM OF CERTIFICATES, NOTATIONS, APPLICATIONS AND FINDINGS.
   The department shall establish the form of all certificates, notations, applications and findings required or permitted by this chapter. All such forms must be approved by the Commission and must be consistent with this chapter.
