(A) If a credit card is lost or stolen the cardholder must immediately notify the credit card company at the toll-free number on the back of the card. In addition, the County Auditor must be notified that the card is lost or stolen on the first available business day.
(B) In order to receive a replacement credit card, the cardholder and his or her department head/elected official must complete a new credit cardholder agreement.
(C) The County Auditor will provide a replacement credit card after the proper forms have been submitted and approved.
