(A) Any person desiring a license shall make application therefore in writing to the Street Commissioner.
(B) The application shall set forth the name and address of the applicant, a description and location of the building or structure to be moved, the location where it is to be moved, and the streets, alleys or other public property over which it is to be moved; also a description of the equipment which will be used in the moving.
