Applications for license shall be made in writing to the Village Clerk upon forms furnished thereby. Each application shall state the name of the applicant, applicant's address, number of individual persons to be operating under such license, type of license requested, and a statement that the applicant agrees to comply with this and all other ordinances relating to or regulating the activities engaged in this chapter.
