§ 112.03 APPLICATION.
   (A)   The application for such license shall contain, but not be limited to, the following information:
      (1)   Name and address of the applicant, age, date, and place of birth.
      (2)   Prior criminal convictions of applicant, if any.
      (3)   Place where machine or device is to be displayed or operated and the business conducted at that place.
      (4)   Description of machine to be covered by the license, mechanical features, name of manufacturer, and serial number.
      (5)   Every application issued under this chapter is subject to the right, which is hereby expressly reserved, to denial of such application to operate any juke box or mechanical amusement device by the Village President and Board of Trustees after written notice to the applicant. Such notice shall specify the reason or reasons for such denial. A hearing may be requested by the applicant. At such hearing, the applicant and his attorney may present and submit evidence or witnesses in his defense.
   (B)   No license shall be issued to any applicant unless he shall be over 21 years of age and a citizen of the United States.
