§ 91.21 APPLICATION FOR LICENSE.
   Applications for the license required herein shall be made not later than 60 days after a person acquires a dog or cat four months old or older or such animal shall attain the age of four months and within 60 days after January 1 of each year for any animal required to be licensed on said date. Applications shall be made to the Village Clerk. The applicant's name and address and the name, breed, color, sex of each dog or cat owned or kept by the applicant, and whether the animal has been neutered, castrated, or spayed, shall be supplied to the Clerk. The applicant shall also deliver to the Clerk at the time of the application a currently valid rabies vaccination certificate, completed by a duly registered veterinarian showing that such animal has been inoculated for rabies, in compliance with this chapter, and that such inoculation is providing, at the time of application, protection from the disease. Upon the foregoing information and certificate being supplied, and the payment of the required fee, the Clerk shall issue the license tag.
