(A) All regular full-time employees of the village are eligible for participation in the village health insurance and life insurance plan.
(B) The premium contribution for each full-time employee of the village or coverage under the insurance plan shall be paid by the village and/or employee in an amount to be determined from time to time by the village. At the election of the employee, his/her dependents may be added to the employee's certificate of insurance or insurance policy as additional insureds.
(C) An employee interested in enrolling himself/herself and/or his/her dependents should do so within the first month of employment. The insurance will not become effective until an employee has been in full- time status for at least 30 days.
