§ 34.08 NOTICES OF EXTENSION AND DENIAL.
   Notices of extension and notices of denial issued on behalf of the village shall be conclusively deemed to have been given if mailed within the applicable time period regardless of the date of actual receipt by the party requesting the records. All such notices shall include a verified proof of mailing in substantially the following form:
      "I              , being duly sworn on oath state that I served the original of the foregoing (Notice of Extension of Time) (Notice of Denial) by mail, addressed to                , at              , this             day of                               , 198  .
