(A) Each applicant for a license to maintain a rental dwelling shall file a written business license application with the village stating:
(1) The full legal name, address and home and work telephone numbers of each and every legal and beneficial owner.
(2) The address of the rental dwelling.
(3) A statement by the owner that service by regular mail upon the owner at the address stated in the application will be considered sufficient service for all purposes.
(B) All license fees shall be due and payable at the time the license application is filed. All licenses shall expire on the first annual anniversary, next after the date of issue.
(C) (1) Each license application shall be accompanied by a fee of $200 per single-family dwelling unit and for each dwelling unit contained in any rental dwelling. If an application for a renewal of an existing license is received after the expiration of the existing license, the license fee shall be increased to $250 per dwelling unit.
(2) The fee includes an annual inspection and one re-inspection if necessary. If more than one reinspection per dwelling unit is required, the fee for each re-inspection shall be $100 per re-inspection per dwelling unit.
(D) Whenever there is a change in the ownership of a rental dwelling or a change in the owner's manager, the owner shall, within 15 days of such change, file a written notice with the village indicating such change including the name and address of the new manager and/or new owner.
(E) Application for license renewal shall be made in the same manner as a new application except that the application shall state that it is for renewal.
