(A) Any not-for-profit group of three or more persons may obtain a fireworks display permit for consumer fireworks by complying with the following procedures.
(1) At least one member of the group shall be resident of the village.
(2) All the members of the group shall be 18 years of age or older.
(3) At least three members of the group shall sign an application form provided by the village which will contain the following information, as a minimum:
(a) The name, address and telephone number of the members of the group applying for the license;
(b) The date(s), time and location of the fireworks display for which a permit is sought; provided, however, that, no displays shall be held other than between the hours of 12:00 p.m. and 12:00 a.m.; and
(c) The name, address and telephone number of the person who will supervise the fireworks display.
(B) (1) Permits for fireworks display shall be issued by the Mayor after the Mayor, the Fire Chief and the Police Chief, or their designees, have inspected the display site and determined that such displays shall not be hazardous to property to endanger any person or persons.
(2) The original permit shall be sealed by the Village Clerk and kept in possession of the supervising person, one copy pf each permit shall be kept on file at the village office, and one copy shall be forwarded by the group to the office of the state’s Fire Marshal, 1035 Stevenson Drive, Springfield, Illinois, 62703.
(3) No fireworks display permit issued hereunder shall be transferable.
(4) Any accident shall be reported to the office of the state’s Fire Marshal within 36 hours.
