(a) Application to Zoning Inspector. An application for a conditional zoning certificate shall be made to the Zoning Inspector on a special form (See Appendix B, “Application Forms”) provided for this purpose. An application fee as required in Section 1149.04 shall be submitted with the application.
(b) Site Plan. A site plan, plot plan or development plan of the entire property drawn at a scale of one inch (1”) equals fifty feet (50), and showing the location of all abutting streets, existing and proposed parking, landscaping, lot dimensions and points of ingress and egress shall accompany the application.
(c) Site Plan Review. The site plan shall be reviewed by the City Planning Commission before a conditional zoning certificate is issued by the Zoning Inspector.
(d) Revocation of Conditional Zoning Certificate. If the applicant, upon receiving approval of a Conditional Zoning Certificate, breaches any safeguard, condition or requirement of this chapter, it shall constitute a violation of this Ordinance, and the Zoning Inspector may revoke such certificate.

(e) Notice to Adjacent Property Owners. Upon receipt of a Conditional Zoning application, the Clerk of Council shall give notice by first-class mail at least ten days prior to the date of the scheduled review meeting, stating the time, place, date and purpose of the review to all owners of property contiguous to, and directly across the street from the parcel or parcels involved.
