1464.03 APPLICATION.
   A person seeking issuance of a permit hereunder shall file an application for such permit with the Building, Housing and Zoning Administrator.
   (a)   Form. The application shall be made in writing upon forms provided by the Building, Housing and Zoning Administrator and shall be filed in the office of the Building, Housing and Zoning Administrator. The application shall set forth, at a minimum:
      (1)   A description of the building proposed to be moved, giving the street number, construction materials, dimensions, number of rooms and conditions of the exterior and interior;
      (2)   A legal description of the lot from which the building is to be moved, giving the lot, block and tract number, if located within the City;
      (3)   A legal description of the lot to which it is proposed such building be removed, giving the lot, block and tract number, if located within the City;
      (4)   The portion of the lot to be occupied by the building when moved;
      (5)   The route proposed by the applicant over which the building is to be moved, preferably designated by use of a map;
      (6)   The proposed moving date and hours including a designation of the applicants estimate of where the building will be in the City at fifteen- minute intervals from the time at which it enters the City; and
      (7)   Any additional information which the Building, Housing and Zoning Administrator finds necessary to a fair determination of whether or not a permit should be issued.
   (b)   Certificate of Entitlement. The applicant, if other than the owner, shall file with the application a written statement or bill of sale signed by the owner, or other sufficient evidence, that he is entitled to move the building. Sufficient evidence shall also be filed by such applicant that the building and the lot from which it is to be removed are free of any entanglements and that all taxes and any City charges against the same are paid in full.
   (c)   Fee. The actual fee for obtaining a permit to move a building over the City streets shall be one hundred dollars ($100.00) for each day or fraction thereof that the building rests on, blocks or impedes traffic on any City street. The applicant shall deposit with the Administrator along with the application a sum equal to one hundred dollars ($100.00) per day for twice the number of days which the applicant estimates the job will take. Unused portions of the fee shall be refunded to the applicant pro rata. Fees in excess of the deposit shall be collected from the applicant in like manner.
