(a) Every application to drill an original well or to re-enter an abandoned well shall be in writing, signed by the applicant or by some person duly authorized to sign the same on his or her behalf, and accompanied by the permit fee. No application shall request a permit to drill more than one well. The application shall include the following:
(1) The name and address of the applicant;
(2) A block map of the twenty acres surrounding the drill site, including the location of the proposed well, distances therefrom to all existing dwelling houses, buildings or other structures designed for the occupancy of human beings or animals within 500 feet of any such well, and the location of all waters of the City and existing oil, gas or fresh water wells within the twenty-acre tract;
(3) A copy of the State drilling permit;
(4) A statement of the proposed drilling schedule, listing when the drilling is expected to begin and to be completed, when the driveway is to be installed, a tentative schedule for the removal of all litter, machinery and deleterious matter accumulated in the drilling operation, and plans showing fencing and landscaping around wells and tanks, with a proposed schedule for completion;
(5) The name or names of those persons directly responsible for the drilling operation and maintenance of the well, listing telephone numbers and addresses where such individuals may be contacted if an emergency arises;
(6) A sworn statement that the owner has in force and will maintain, until abandonment of his or her oil or gas well in the City, liability insurance listing the City as an additional insured, in an amount of not less than three hundred thousand dollars ($300,000) bodily injury coverage and one hundred thousand dollars ($100,000) property damage coverage, to pay damages for injury to persons or damage to property caused by the drilling, operation or plugging of each well; and
(7) A copy of a restoration plan for the restoration of land surfaces, as required by Ohio R.C. 1509.06(M), and, where applicable, a land owner waiver form.
(b) Where the application is one for re-entry of an abandoned well, such application shall contain all the information set forth in subsection (a) hereof, and shall provide the following:
(1) A statement of the condition of the well; and
(2) Evidence of adequate current tests showing that the casing strings currently passed the same tests that are required in the case of the drilling of an original well.
(c) A copy of a spill prevention control and countermeasure plan shall be filed with the City for storage facilities having a capacity of 1,320 gallons of oil or single containers in excess of 660 gallons.
