The city clerk shall not issue a permit hereunder until he has received an application therefor duly approved by the city engineer or such other person to whom that responsibility may be delegated by the city manager. The application for permit must be made in writing upon forms furnished by the city. The application shall contain the name and address of the person, firm or corporation making the application, the name of the contractor or person who is to construct the driveway, and the proposed location and dimensions of such driveway. Plans and specifications shall be attached to and made a part of the application. (Ord. 982 § 2, 1960)
