(A) Form 101, dog tag statement, shall include the name and address of the applicant, description of the dog or dogs, dog tag number and the appropriate fees.
(B) Whoever becomes the owner or harborer of a dog after the dog census by the Township Assessor, or any owner or harborer of a dog for which, for any reason, the Township Trustee failed to collect the dog tag tax, shall, within 30 days after becoming the owner or harborer of a dog, apply to the Township Trustee and pay the required fee and procure a tag for the dog or dogs.
