(A) Application. The application for a license shall contain the following information:
(1) The name and address of the applicant; the applicant’s age, date and place of birth;
(2) All prior convictions of felonies of the applicant, if any;
(3) The address and the name of the business where the machine or device will be displayed and operated and the nature of the business conducted at the address and under the name; and
(4) The name and address of the owner of the machine, and, if the machine is serviced and supplied by any person, firm, corporation or association other than the applicant or the owner of the machine, the name and address of such person, firm, corporation or association.
(B) Qualifications. No license shall be issued to any applicant unless he or she shall be over 21 years of age.
