Every person receiving any payments for admission, dues or fees, taxable under this chapter shall, on or before the twenty-fifth day of each calendar month, make a return under oath to the Finance Director on such form as the Finance Director may prescribe, showing the number of taxable admissions issued and disposed of and the amount of taxable dues and fees collected during the preceding calendar month, the amount of tax hereby imposed on the same and such other facts and information as the Finance Director may require in the forms of return prescribed by him.
