6-2-5: PERMIT REQUIREMENTS:
   A.   Permits Required; Work Without Permits:
      1.   No electrical equipment shall be installed or altered except upon a permit first issued by the electrical inspector authorizing the installation, alteration, use or repair of electrical equipment.
      2.   Where an electrical installation has been started prior to the issuance of a permit for such work, the permit fee for such work shall be twice the amount of the normal permit fee prescribed in this chapter but no permit fee shall exceed the normal permit fee for such work plus two hundred fifty dollars ($250.00).
      3.   Where a registered electrical contractor is found doing electrical work without a permit on three (3) separate occasions in one calendar year, a hearing shall be held by the director of the office of quality control, at which time the permit privileges of said electrical contractor may be suspended for a period of time not to exceed one year.
   B.   Application For Permit:
      1.   No permit for the installation, alteration, repair or use of electrical equipment shall be issued except pursuant to an application therefor made out on a printed form, to be furnished by the electrical inspector, or his duly authorized deputy, and upon payment of a fee in advance as hereinafter provided.
      2.   The application for each permit shall contain the name of the owner, or user of electrical equipment to be installed, altered, repaired or used, and the location of the premises where such installation or alteration is to be made by a street and number and a detailed description of the work to be done.
