§ 94.15 REPAIR OF GRADE CROSSINGS.
   (A)   When work on a grade crossing is anticipated, a letter of application for authorization shall be submitted to the village with appropriate scaled plans and profile drawings, as per R.C. § 5561.16. One copy of the application and two copies of the plan will be required.
   (B)   A traffic control note must be included that "Type 3" barricades will be erected at the crossing and at the first crossroad on each side of the crossing in accordance with the "Ohio Manual of Uniform Traffic Control Devices".
   (C)   The application will be reviewed in the office of the Village Administrator. The review will include the issuance of appropriate instructions and conditions based on individual circumstances or conditions. Conditions of the authorization will normally refer to a list of notes common to this type of situation. Exceptional situations will be dealt with separately. The railroad should allow ample lead time in planning repair work to allow for processing of authorization, normally seven to ten days of receipt of the application to the date of issuance.
   (D)   The village has a bench mark, a chiseled "+", on the north bonnet bolt of the fire hydrant on the east side of Locust Street, between Bank and South Railroad Street, elevation 590.86. Information is available in the office of the Village Administrator.
   (E)   Any further survey that would have to be done, when Con-Rail is maintaining or repairing the grade crossing on State Route 19 (Locust Street) and on Benton Street to satisfy the village as to the height of the crossing, will have to be done at the expense of Consolidated Rail Corporation.
   (F)   In addition to the village providing instructions on the authorization dealing with the actual construction or repair procedure, the village will also provide instructions as to whom to notify prior to beginning work. It is also expected that the Railroad will provide the name and telephone number of a local contact, an individual who is directly responsible for the activity involved. The Railroad should also provide instructions as to what address approved authorization should be mailed to. The work crew must have the authorization in hand prior to beginning work.
