All applications for a bicycle registration, or for the transfer thereof, shall be made in writing to the City Clerk or his or her designee, on forms to be furnished by the City Clerk, which shall state the make, serial number, model and description of the bicycle to be registered, the name and residence address of the owner of the bicycle, the signature of the owner, the owner’s age, and, if the owner is under 18 years of age, the name and address of his or her parent or guardian, the name and address of the person from whom the bicycle was purchased and the date of purchase.
