§ 111.04 LICENSE APPLICATION; DESIGN PLANS; SCHEDULE OF EVENTS.
   (A)   An application for a food service establishment license, retail food store license, temporary food service establishment license or occasional food service establishment license shall be made in writing to the City Clerk, and the appropriate license fee, as provided by ordinance shall accompany the signed application. The application shall be made on a form provided for that purpose.
   (B)   (1)   The form shall contain a notice that as part of the procedure an inspection will be conducted. The form shall also contain a statement that the applicant grants permission to enter the premises, during reasonable hours prior to the license being issued, to conduct the license inspection.
      (2)   The form shall include notice that at least once every six months, the city’s Health Commission shall make an inspection of the applicant’s establishment. The applicant’s grant of permission to enter shall be in accordance with that schedule.
      (3)   In the case of a new establishment or remodeling of an existing structure, the applicant shall submit final design plans to the city’s Health Commission for approval. An inspection of the completed premises will be conducted by, and approval shall be obtained from, the city’s Health Commission before the issuance of a license.
   (C)   When an occasional food service establishment has a regular schedule of events, the schedule of events must be attached to the application. The schedule may be modified or added to, provided that written notice of the modification or addition is submitted to the city not less than five days prior to the event.
