§ 90.120 REQUEST.
   (A)   Every request for the honorary designation of a street shall be presented in writing to the City Clerk.
   (B)   All requests must include the following information:
      (1)   Name of the person, organization or entity to be honored;
      (2)   The humanitarian, civic, community or other reasons which make the honor appropriate; and
      (3)   The name, address and phone number of the person submitting the request.
   (C)   The request shall also be accompanied by either a petition signed by occupants of at least 75 residential locations within the city or a memorandum submitted by at least three aldermen or the Mayor.
