§ 120.04 CONTRACTOR REGISTRATION APPLICATIONS.
   (A)   Application for contractor registration shall be made in writing to the City Clerk or a designee on a form provided by the city.
   (B)   Each application shall state:
      (1)   The date of submittal of the application to the city.
      (2)   The name, business address, telephone number and e-mail address of the applicant.
      (3)   Whether the applicant is a general contractor, public utility contractor or a trade contractor.
      (4)   If the applicant is a general contractor or a public utility contractor contracting with trade contractors, the name, business address, telephone number and e-mail address of all trade contractors performing work with whom the general contractor or public utility contractor is contracting and identification of the type of trade work being performed by the trade contractor.
   (C)   Each application shall be signed by the applicant if the applicant is a person or if the applicant is a partnership, corporation or other business entity by an authorized agent of the applicant and shall identify the authorized agent's name, title, office or authorized agency with the applicant.
   (D)   Each application shall be accompanied by the following:
      (1)   The applicable registration fee.
      (2)   Elevator contractors, fire sprinkler system contractors, private sewerage disposal contractors and roofing contractors must possess and submit a copy of their current license issued by the state of Illinois.
      (3)   A surety bond issued by a company approved and authorized to issue bonds by the state of Illinois. The surety bond shall assure that the contractor's work will be completed in accordance with the requirements of the building permit and the city's code. The surety bond shall be in the amount of $10,000 and list the city and third-party beneficiaries as the obligees. Contractors completing stormwater work under a stormwater permit are exempt from the requirement to provide a security bond under this section.
      (4)   A certificate of insurance evidencing the insurance coverages required by § 120.05.
