6-6-2: CONTENTS OF APPLICATION:
The applicant for such permission shall file an application in writing with the city manager, which shall clearly indicate:
   A.   The name of the person, firm or corporation which will use the structure or sponsor the event.
   B.   The location of the principal place of business of such person, firm or corporation.
   C.   The names and addresses of the officers of such firm or corporation.
   D.   The length of time the structure is intended to be used for the purpose for which applied.
   E.   The hours of the day or night during which such structures are intended to be used as a place of assembly or during which the event is to be conducted.
   F.   Such other relevant information as the city manager may require. (1962 Code § 5-1-35)
