(a) The Board of Health shall prepare certificates in triplicate form, together with a corresponding dog tag for distribution to the licensed veterinarians. The certificate shall contain the following information to be inserted thereon by the veterinarian and such other information as the Board of Health may require:
(1) Name and address of the owner or harborer of the dog,
(2) Date of vaccination of dog and number and year of corresponding dog tag;
(3) Breed, age, color and sex of dog.
(b) The Board of Health shall, at the time of issuance of certificates to veterinarians, also provide a corresponding tag of durable material to be attached to the collar or harness of the dog as evidence of inoculation with anti-rabic vaccine. The tag shall be numbered and contain the year of issuance. The tags and certificates shall be purchased by the veterinarians from the Board of Health at a cost of one dollar ($1.00) for each certificate, in triplicate, and corresponding tag.
(c) The Board of Health is authorized to refund to the veterinarian the cost of any unused tags or certificates returned to it before January 15 following the year of issuance.
