959.02 APPLICATION FOR TERMINATION.

Formal application for each alarm service termination desired at the Communications or Police Department shall be submitted to the Communications Officer at the Communications Center, 4725 Montgomery Road, Norwood, Ohio, 45212. Only upon approval of the application and payment of fees, shall the alarm termination and installation of the lines be permitted.
