933.06 APPLICATION FOR PERMIT.
   All applications for permits must be in writing upon blanks furnished by the Director of Public Service, signed by the owners and tenants of the premises, for whose benefit it is made, or by his or their authorized agents or attorneys and must state the number of the lot, number of feet front, name, number or letter of the sewer district in which the premises are located, the name of the owner, and the number and character of the buildings to be connected and how occupied, the number of drains required and the water closets, sinks and other fixtures to be connected with the same. The applications shall be filed in the office of the Director of Public Service.
