767.03 APPLICATION; PROCEDURE.
   (a)   Every applicant for a license as a driver of a taxicab or auto for hire shall make
application to the Director of Public Safety on forms to be supplied by the Director. The application shall set forth that the applicant is:
      (1)   A citizen of the United States;
      (2)   Able to speak, read and write English;
      (3)   Eighteen years of age or over;
      (4)   Free of defective vision, defective hearing, epilepsy, vertigo, heart trouble and any other infirmity, physical or mental, which would render him unfit for safe operation of a public vehicle;
      (5)   Not addicted to the use of alcohol or drugs and such other pertinent information as the Director may require.
   (b)   Applications shall be sworn to by the applicant.
   (c)   The application form shall further contain the certificate of a reputable physician of the City stating that the applicant is free from infirmities which would prevent his driving a public conveyance with safety.
   (d)   The applicant shall file with his application two photographs of himself of a size designated by the Director, which photographs shall have been taken within thirty days next preceding the date of the application. (A.O.)
   (e)   The applications will contain a release permitting the Director of Public Safety or his employees or agents to check the applicant’s traffic and criminal records with any Clerk of Courts or Bureau of Motor Vehicles. The applicant shall file with his application a photocopy of his driver’s license.
