Every applicant for a barber's license shall exhibit an Ohio Registered Barber's Certificate issued to the applicant and file with the City Auditor a written application stating the full name and address of the applicant including the street and number, if more than one person or if an association or corporation, the full name and addresses including street numbers of all parties interested. The application shall also state the following:
(a) The exact location, including street and number where the barber shop is to be opened, conducted, maintained or operated.
(b) Whether the applicant or applicants are citizens of the United States.
(c) The name and address of the owner of the premises in which the barber shop is to be located.
(d) The name under which the barber shop will be opened, maintained, operated or conducted, and the application shall be signed by the applicant.
