(A) The initial statement of financial interest required by this section shall be filed with the Ethics Officer no later than 4:00 p.m., April 15, 1995.
(B) All subsequent statements of financial interest shall be filed no later than 4:00 p.m. on April 15 each year, provided that:
(1) An officer or employee newly-appointed to fill an office or position of employment with the city or city agency shall file his or her initial statement no later than 30 days after the date of appointment;
(2) A candidate for city office shall file his or her initial statement no later than 30 days after the date on which the person becomes a candidate for elected city office;
(3) The Ethics Officer may grant a reasonable extension of time for filing a statement of financial interest for good cause shown; and
(4) In the event there is a material change in any information contained in a financial statement that has been filed with the Ethics Officer, the officer or employee shall, no later than 30 days after becoming aware of the material change, file an amended statement with the Ethics Officer.
