856.04 APPLICATION INFORMATION.
   (a)   The original and renewal application(s) for a skill-based amusement arcade license and the license tag for each skill-based amusement device or game machine shall be submitted on forms provided by the Administrative Officer. The application shall set forth therein information such as the name and address of the operator, the address of the place of business which is to be licensed as a skill-based amusement arcade, the year for which the license is sought, the number of skill-based game machines or amusement devices located at such place of business, the name and address of the owner of each machine, a detailed explanation of the machine operation, applicable software license authorization, source codes, player skills, and training required qualifying each machine as a game of skill, and such other information as the Administrative Officer reasonably deems appropriate. The operator in whose name the City licenses and license tags are to be issued as well as the owner of the real property shall sign the application.
   (b)   The original and each renewal application shall be accompanied by a certificate or report, from an agency approved by the Ohio Attorney General's Office's Bureau of Criminal Identification and Investigation (BCI), listing the results of a criminal background check for each manager, supervisor, employee and owner of the applicant. Prior to or upon application, each manager, supervisor, employee, and owner shall report for fingerprinting and submittal of a request for a background check. All costs associated with securing this pre-application background check are the sole responsibility of the applicant.
   (c)   Each applicant submitting an original application for a skilled-based amusement arcade license shall also submit a non-refundable application fee equal to thirty dollars ($30.00) at the time such application is submitted.
