(A) The above connection charge shall be payable upon application for a permit to make such connection. The above maintenance and billing charge shall commence on the date the connection is made.
(B) All bills shall be rendered in calendar quarters in advance and shall be subject to a 5% penalty if not paid within 30 days after the date of the bill and to a further 1% penalty per month on any balance remaining unpaid after 90 days from the date of the bill.
