Any application for any beer license shall be verified and filed with the city recorder. The application shall state the full name, address, telephone number and date of birth of the applicant or applicants if individuals, the names of the partners if a partnership, and the names of the directors and officers if a corporation. The applicant shall show evidence that he has complied with the requirement and possesses the qualifications required in the state alcoholic beverage control act and the application shall show that the applicant has read and understands the provisions of the act and the business and beer ordinances of the city.
