The site plan shall be prepared by a registered surveyor or engineer at a scale of one (1) inch equals fifty (50) feet of the entire boundaries of the project, and at a minimum the entire parcel subject to any grade change, shall be displayed on a single sheet when possible. All engineered drawings shall contain the following minimum information:
(a) The name and address of the applicant and a brief description of the proposed change of grade.
(b) The name and address of the engineer or surveyor, a north arrow, scale and date.
(c) The location and dimension of all lot lines or boundaries within one hundred (100) feet of the proposed area subject to a change of grade.
(d) The current zoning of the property affected by the grade change and the name of the owner.
(e) The location and dimensions of all boundary lines of the property expressed in feet and decimals of a foot.
(f) The location of existing water bodies, ponds, lakes, streams, drainage ditches, drainage systems, and other appurtenant features including the direction of natural drainage.
(g) An outline of all structures within one hundred (100) feet of the proposed grade change with approximate dimensions of each structure and the distance to the existing structure.
(h) The excavation or fill areas of the proposed grade change showing the amount of fill or excavated material in feet and decimals of a foot from the natural grade. Where this condition shall extend for more than fifteen (15) feet horizontally, measurements of the excavation or fill shall be shown every fifteen (15) feet to the ends of the fill or excavated area showing the extent of the finish grade.
(i) All proposals for relocating storm sewer, water supply and sanitary sewer systems and for the collection and discharge of all water drainage including the location and size of existing and proposed systems, water mains, sanitary sewers, tap tees, risers and drainage facilities.
