Application for registration of a bicycle shall be made to the Chief of Police upon forms provided by him. Such application shall contain the name and address of the applicant, the name and address of the owner's parents or guardian in the event the applicant is a minor, the make, color and serial number or other identifying marks of the bicycle and other information as the Chief of Police shall deem pertinent.
