(A) Each driver of a taxicab shall provide a passenger with a receipt when requested.
(B) The receipt shall include complete and accurate information in legible form as follows:
(1) The name and signature of the drive and the individual taxicab license number;
(2) Date, time and place of trip origin and dismissal; and
(3) Itemized listing of charges and total fare paid or charged.
