4-5-3: PERMIT TO CONSTRUCT OR ALTER PRIVATE SYSTEMS:
   A.   Permit Required: It shall be unlawful for any person to construct or make alterations to individual sewage disposal systems within the Village unless he holds a valid septic permit issued by the Health Officer in the name of such person for the specific construction or alteration proposed. The application forms for permits are available at the Village Hall. All new septic systems, major repairs or modifications to such systems shall be constructed only after the issuance of a permit from the Village. The permit issued by the Health Officer is in addition to any building permit which may be required and must be obtained prior to application for the building permit. The permit for construction of any sewage disposal component shall be considered upon submittal of the appropriate application. No percolation tests are required for repairs to existing septic systems. (1977 Code § 7-4-2; amd. Ord. 854, 11-20-2000; 2004 Code)
   B.   Plot Plan: With the septic permit application set forth in subsection A of this section, the applicant shall furnish to the Village Health Officer six (6) copies of the site plan with the septic layout included and as described.
   C.   Contents Of Application: The application shall contain a general description of the work. The site plan must include the following information:
      1.   The scale of the plan, which shall be no smaller than one inch equal to thirty feet (1" = 30'). (1977 Code § 7-4-2; amd. 2004 Code)
      2.   For the construction, repair, or alteration of an individual sewage disposal system, there shall be provided topographic information shown at one foot (1') contours and extend approximately fifty feet (50') beyond the property lines unless the Health Officer determines in writing that topographic information is not required. NGVD88 datum (bench mark) is required on all plan submittals and shall be referenced to the Illinois state plane coordinate system.
The base plan which includes existing topographic information, physical features and other information shall be computer generated to enhance accuracy. An electronic copy and four (4) prints shall be made available upon request by the Village.
Record drawings of completed construction are required for all new construction and other site modifications which require grading plans. An electronic copy (CD) in AutoCAD format on the same datum stated above and three (3) prints, signed and sealed by a registered professional engineer, shall be provided to the Village prior to final approval.       3.   A fixed bench mark which is not likely to be destroyed during construction.
      4.   A proposed grading plan to include:
         a.   All contour line changes.
         b.   Top of foundation.
         c.   Water level of lakes, ponds, streams and other bodies of water.
         d.   Location and finished grade of driveway at several locations.
         e.   Portion of the property which is adjacent to the edge of the street bordering the property unless waived in writing by the Health Officer.
         f.   Location, size and invert of proposed culverts.
         g.   Bottom of front ditch or swale at several points.
         h.   Curb and gutter elevation.
         i.   Rim and invert elevations of all storm sewers and structures.
         j.   All new swales.
      5.   House footprint with dimensions and setback measurement from property lines.
      6.   The location of other proposed structures such as decks, outbuildings, pools and other constructed improvements.
      7.   Number of bedrooms.
      8.   Easements.
      9.   Property dimensions.
      10.   Required setbacks.
      11.   Location and identification of any existing structures on the property.
      12.   Proposed well location.
      13.   All wells within seventy five feet (75') of the property line.
      14.   Location of soil test holes.
      15.   Any existing trees of six inch (6") caliper or greater measured four feet (4') above the existing ground surface.
      16.   All components of the septic system and the following information:
         a.   Invert of house sewer at foundation. The pipe must be laid a minimum two percent (2%) slope;
         b.   Inverts of all pipes through the tank system and entering the disposal system;
         c.   Inverts of header line and seepage tile at each box;
         d.   Invert of curtain drain at both ends;
         e.   Trench, curtain drain and lift station details;
         f.   Length and sizes of pipes and material type;
         g.   Location, extent, type and depth of any required fill;
         h.   Description of fill process; and
         i.   Other information as may be required by either the Building and Zoning Officer or the Health Officer.
      17.   Said drawings shall have been prepared, certified and approved by a professional engineer, registered in the state of Illinois, experienced in sanitary engineering. (1977 Code § 7-4-2; amd. 2004 Code)
   D.   Guarantee Of Performance: Prior to the issuance of any permit herein, the applicant shall furnish, for the benefit of the Village, a cash deposit, a letter of credit or a performance bond in the amount of two thousand dollars ($2,000.00) for each such individual sewage disposal system or major alteration thereto. The condition of said deposit, letter of credit or bond shall be that all requirements of this title and any other ordinances or state statutes shall be complied with in connection with the installation of or major alteration to said individual sewage disposal system. For new home construction, said bond shall be valid until the later of: 1) one year from the date of issuance of final occupancy permit for the building, or 2) one year after continuous occupancy of the building served by the sewage disposal system, or 3) two (2) years after the date of filing the completed septic permit application. For septic repairs, the bond shall be held for one year from date of approval of installation. The dates for the bond expiration may be extended by the Health Officer if there are unforeseen circumstances. It will be the responsibility of the applicant to renew the bond if appropriate or face additional charges.
   E.   Permit Fees: Permit fees for individual sewage disposal systems shall be as follows:
