717.04 INVESTIGATION.
   The Village Administrator or his authorized representative shall investigate each application and inspect each vehicle to be used for hauling under this chapter, to determine the suitability of such applicant and each vehicle to render the services of a hauler. Applicant shall furnish and file with the Village Administrator or his authorized representative the following:
   (a)   The name of each person in the employ of the hauler who will be performing any work within the Village or who is to be authorized to operate the vehicle or vehicles being licensed.
   (b)   Proof of liability insurance in effect for the period of time of the license applied for, or longer, issued by a responsible insurance company, approved as to sufficiency by the Village Administrator or his authorized representative, and as to legality by the resulting from the operation of each vehicle to the extent of one million dollars ($1,000,000), on account of injury or death of each person in any one accident; three million dollars ($3,000,000), on account of injury or death of more than one person in any one accident; and one million dollars ($1,000,000), for property damage caused in any one accident. Proof shall be required that the insurance coverage extends to all vehicles being licensed.
   (c)   Proof of compliance with all of the rules and regulations of the Wood County General Health District, including proof displayed to the Village Administrator or his authorized representative that a valid, current license was issued by said County District effective on or before June 30 of said year, authorizing said hauler to engage in garbage, rubbish, refuse, and solid waste hauling in the County.
   (d)   Proof that each driver or operator of any vehicle used for trash hauling has a current valid commercial driver’s license (CDL) issued by the Registrar of the State of Ohio, if the vehicle to be licensed is of the type requiring an operator with a C.D.L.
   (e)   Each vehicle to be licensed shall be subject to inspection by the Police Chief of the Village or his designated agent to determine that said vehicle meets all safety requirements for vehicles of that particular size or type as set forth in the Ohio Revised Code, and/or the Codified Ordinances of the Village of North Baltimore. Said inspection shall include, but not be limited to, the following items; lights; horn; service brakes; parking or emergency brakes; steering; tire tread; stone guards or mud flaps. (Ord. 40-94.
