(a) All applications for permits for water or sewer taps must be made in writing to the Village Administrator or by the owner of the real estate for whose benefit the application is made, or by his agent authorized in writing to make said application, and shall be accompanied by the fee specified in the Village Water, Sewer and Tapping Permit Rate Schedule, to defray the cost of issuing such permit and the expenses of making the tap connection and/or supervising such connection. In addition, if a curb cut is required to install the requested utility service line an additional fee as specified in Village Water, Sewer and Tapping Permit Rate Schedule must accompany the application.
(b) The application shall state the location, number and description of the lot or lots to be served by said connection, the number of front feet, the name and address of the owner of said property, the number of buildings to be connected, the number of drains to be served by said connection and the number of water closets, sinks, tubs, showers, laundries and other water use facilities, or devices to be serviced by said tap or taps. The application shall state whether a curb cut is required for the installation of either the water or sewer tap or both. The application shall be made on the form prescribed by the Village Administrator.
