(A) An application for a license filed with the Clerk-Treasurer shall include all information required by this chapter and shall be submitted to the Board of Public Works and Safety for consideration. The application shall include the street address of the location of the applicant's business operation.
(B) The application shall include a schedule of rates and fares to be charged for by the applicant.
(C) The application shall include the names, drivers licence numbers, and dates of birth of all persons to be drivers of the taxicabs. After the granting of the license, the applicant shall continue to provide the city with the names, drivers licence numbers, and dates of birth of any additional operators of taxicabs to be operated by the applicant. All drivers license numbers and dates of birth shall be submitted in a separate sealed envelope and shall only be used for such background checks as the city deems appropriate. Such information shall be maintained as a confidential record, not subject to public disclosure under Indiana law.
(D) Each application shall include a non-refundable $100 fee.
