§ 72.86 INFORMATION REQUIRED IN APPLICATION.
   The application form shall contain the name, and address, telephone number, and email address of the applicant along with the year, make, model, color, and license plate number of the automobile(s) owned by the applicant. Downtown employees or business owners shall also include the name, address, telephone number, and work email address of their workplace. Applicants shall show a picture I.D. in which a copy will remain with the permit. Only the person who is issued the parking permit shall be able to renew the permit, and again, a photo I.D. valid driver's license will be required at the time of renewal.
