§ 159.123 RESIDENTIAL CARE HOMES.
   Residential care homes shall be a permitted use in all residential districts, as permitted by Indiana law, and provided that such uses shall be registered with the Department of Planning and Development. Such registration shall include the following:
   (A)   Name, address, and phone number of owner.
   (B)   Name, address, and phone number of operator.
   (C)   Names, addresses, and phone numbers of individuals who may be contacted in an emergency (24 hours).
   (D)   Address and phone number of the home itself.
   (E)   Number of residents.
   (F)   Disability of the residents.
   (G)   Any special information concerning special needs or limitations in case of an emergency (e.g. impaired mobility, impaired hearing or vision).
   (H)   Evidence that all required licenses, certifications, permits, and approvals are up to date and maintained.
