§ 31.017 APPLICATION FOR PLAN INVESTMENT.
   As a condition to participation in the plan, the participant shall complete the application form(s) provided by the issuer(s) of any plan investment(s) in order for the appropriate contract(s) or certificate(s) to be issued or record established.
