Application for a license to display a mechanical amusement device must be made to the City Manager upon such forms as shall be prepared therefor by the City Manager. Such application must be made by the owner or proprietor of the business or place at which such mechanical amusement device is to be displayed. Application shall state the name of the owner of such place or business, the address of the place for which the license is applied, the residence address of the owner, the nature of the business in conjunction with which such mechanical amusement device is to be displayed, together with such other and further information as may be required by the City Manager.
