Applications for the license for street vending shall be filed with the City Clerk on a form furnished by her. The form shall require at least the following information:
(a) Name of applicant;
(b) Location of temporary site of business;
(c) Length of time business will be conducted in the City of Newton Falls;
(d) Nature and character of goods, merchandise or food to be sold;
(e) Written permission of the land owner of the site in which you will be located;
(f) Applicant's social security number.
(g) All appropriate licenses.
(h) Proof of Federal Tax ID number.
(i) Provide proof applicant has registered with the City Income Tax Department.
(j) Proof of insurance and/or bond.
