§ 120.05 REGISTRATION APPLICATION.
   (A)   The owner or manager of any rental property intended to be let or currently being let shall register the rental property with the city within 30 days of the effective date of this chapter. For 2008 and 2009, all rental property registrations shall expire December 31, 2009. Thereafter, the registration shall be effective for a two year period as of January 1 of the even year and shall expire December 31 of the following year.
   (B)   The registration application shall be available at City Hall and shall be filed with the City Administrator.
   (C)   A registration application must be filed with the City Administrator at least 30 days prior to any conversion of a dwelling to use as rental property.
   (D)   Each registration application shall contain the following information:
      (1)   Rental property building type;
      (2)   Name of rental property complex, if applicable;
      (3)   Address of the rental property;
      (4)   Property identification number of the rental property;
      (5)   Number of rental property units described by the number of bedrooms and other units;
      (6)   Name, address, and a description of the legal entity owning the rental property;
      (7)   Name, address, and telephone number of a contact person and manager of the rental property; and
      (8)   Owner’s signature acknowledging receipt of the requirements described in the City Code of Ordinances and attesting that the rental property will be operated and maintained according to the applicable requirements and subject to the applicable penalties for violations.
   (E)   If the proposed use of the dwelling does not comply with the city’s zoning code, then the registration application will not be accepted.
