Applicants for a license under this chapter must file with the Clerk-Treasurer a sworn application in writing, in duplicate, on a form to be furnished by the Clerk-Treasurer which shall give the following information:
(a) Name and description of the applicant;
(b) Permanent home address and full local address of the applicant;
(c) A brief description of the nature of the business and the goods or services to be sold;
(d) If employed, the name and address of the employer, together with credentials establishing the exact relationship;
(e) The length of time for which the right to do business is desired;
(f) If goods or property is being offered for sale, the place where the goods or property proposed to be sold, or orders taken for sale thereof, are manufactured or produced, where the goods or products are located at the time the application is filed, and the proposed method of delivery;
(g) If services are being offered for purchase or hire, the identity of each person, company or other entity who or which will actually provide such services, along with a copy of all licenses permitting each such person, company or other entity to offer and perform such services;
(h) If the vending is take place from a fixed location, evidence of the property owner’s permission to the applicant allowing such vending;
(i) The days of the week and the hours during each day that the applicant requests to perform the soliciting or canvassing;
(j) A recent photograph of the applicant and of each agent who will participate in the soliciting or canvassing;
(k) A statement as to whether or not the applicant or agents has been convicted of any crime, misdemeanor or violation of any municipal ordinance, the nature of the offense and the punishment or penalty assessed therefor.
At the time of filing the application, a fee of ten dollars ($10.00) shall be paid to the Clerk- Treasurer to cover the cost of investigation of the facts stated therein.
