Members will be responsible only for an amount actually paid by a member’s insurance company, under the following conditions: if applicable insurance pays a portion of the charges, then the member is not responsible for any balance not paid by the member’s insurance. If the member is uninsured or the member’s insurance pays nothing toward the fee, or pays and amount equal to, or less than, a member’s deductible, the fee would be charged to the member at 70% of the total fee.
