(a) Applications for licenses for the emergency transportation of patients shall be filed with the Mayor on a form to be furnished by the Mayor, which shall require, at least, the following information:
(1) The name of the applicant.
(2) The type of entity of the applicant (e.g. individual, partnership, corporation).
(3) The address of the applicant.
(4) The names of one or more responsible persons, officers, etc., if the applicant is other than a natural person.
(5) A statement that the applicant has complied with the requirements of Ohio R.C. Chapter 1716 pertaining to charitable solicitations, if applicable.
(b) All applicants shall provide to the Mayor, with their application proof that:
(1) They have sufficient certified personnel and an adequate plan of operations, to be able to insure that at least two currently certified Emergency Medical Technicians, as provided in Ohio R.C. 4731.82 et seq., will be available and present during the transportation of patients.
(2) They are currently authorized under State law to transport patients.
(3) They have met the insurance coverage requirements set forth in Section 717.06.
(c) All applicants must enter into a written contract with the Village prior to beginning operations. The contract may require representation by the Village on the Board or policymaking body of the applicant. Failure to comply with the terms of the contract shall be grounds for the revocation of the applicant's license.
(d) Revocation of a license, prior to its natural term, shall be effective upon the mailing of written notice, by the Mayor, to the address of the applicant given in the applicant's application. (Ord. 10-82. Passed 3-15-82.)
